After clicking the Complete profile (2 min) button, you will be required to complete several documents.

A total of five sections are included in the documents: Personal and contact information, Racial Origin, Medical Background, General Readings, and Lifestyle. You must fill out the forms and provide all the authentic information about yourself. After completing every section, you must press the Save button otherwise your data will not save and you won't go to the next page. Once the profile data has been completed, you will receive a screen informing you of your completion.

Upon tapping I'll do it later, a message box will appear. By pressing Do Later, you will be presented with another message box.

Another message box will appear with two options: Remind me tomorrow and Remind me in three days.

Step: 3

Simply click on any of the options and the reminder will be set accordingly.

If you want to complete the profile later, go to the profile section of the CLH App and complete the profile accordingly.

Tips:

  • If you don’t complete your profile, you will remain in the Pending Activation list. Profile completion is important for successful self-onboarding.

  • Understanding both race and ethnicity is crucial in healthcare, as they may influence susceptibility to certain diseases. Race primarily pertains to physical traits, while ethnicity relates to cultural upbringing and associated factors. By considering both, doctors can enhance their ability to assess and deliver optimal medical care tailored to individual needs.
